You can contact them directly if you are having trouble with any aspect of your benefits. They provide advocacy help and can often resolve problems for you.
Retirees enrolled in Kaiser can email the District Benefits Office at benefits@peralta.edu or call 510-466-7229 for advocacy help.
As a result of the changes in benefits that occurred on January 1, 2023, Peralta has committed to implementing a reimbursement policy and procedure for retirees whose benefits were reduced due to those changes. This includes coverage for:
· providers who do not accept Medicare Assignment
· benefits covered by Peralta medical insurance prior to the change, but not currently covered
· prescription benefit co-pays that exceed the amount specified by the contract in place on the date of retirement
· coverage while abroad not covered by the current policy but that would have been covered prior to the change
· hearing aid coverage for Kaiser members
· any other expense that would have been covered prior to the change and is not now covered either partially or in full
If you have such an expense, you can submit a Request for Reimbursement from the District.

Medicare Claim Form: If your provider accepts Medicare Assignment they should bill Medicare. If they do not accept Medicare Assignment, you will have to pay for the service provided and submit a Request for Reimbursement from the District. The District may require evidence that Medicare did not pay the claim. You can submit the bill to Medicare directly using the Patient’s Request For Medical Payment Form which is available here.
