Note: If you are submitting a form requesting reimbursement
through Trustmark for a payment to a provider who is out of network,
we recommend submitting the Trustmark (rather than the Amnthem) form.
The form, however, should be mailed to the Blue Cross processing center:

Trustmark has its own website with many features including
a screen to order more ID cards, and check on the status of your own
medical claims. You will be able to select a user ID and password
so that you can access your personal Trustmark information at any
time.
